Register for Camp
There are two ways to register for Camp. Complete registration online or request a paper application.

Online Registration
Step 1: Download the Supplemental Camper Application Forms from of the Camp to which you are registering (these are forms that need a signature and cannot be completed online).
- Read through all the Supplemental Forms.
- Have the child’s parent/guardian or appropriate health care provider complete and sign the supplemental forms.
- Send the signed forms via U.S. Postal Mail to your local American Diabetes Association as soon as possible along with the non-refundable deposit if not paying online with a credit card.
Step 2: Go to the online application.
- Complete the remainder of the application online.
- You can choose to pay the non-refundable deposit, the full camp fee or both online with a credit card.
- For technical support, contact your local American Diabetes Association.
- Your child’s application is not complete until the signed Supplemental Forms and payments are received by your local American Diabetes Association.
Step 3: Check your mail for a confirmation notice and information packet from your local American Diabetes Association.
- Applications are processed in the order they are received.
- Once received, review the information carefully. Check for additional information needed from you, deadlines for payments and paperwork submission, check-in information, packing list, and other Camp details.
- If you do not receive a confirmation in two weeks, contact your local American Diabetes Association to check the status of your application.
- Acceptance is contingent upon review of all camper forms by the Camp Medical Team to ensure that we can accommodate your child’s needs and provide a safe environment in which they can fully participate in the Camp program.
Paper Registration
Step 1: Download the Camp Brochure.
- Print the Camp brochure.
- Fill out the reservation form.
- Write a check for the non-refundable deposit or complete the credit card payment section on the reservation form.
- Mail the reservation form and non-refundable deposit to your local American Diabetes Association.
- This reserves a space for your child but does not complete the registration process for Camp.
Step 2: Check the mail for the Camp Application from the American Diabetes Association.
- After the American Diabetes Association receives your reservation form and deposit, a paper application packet will be mailed to you.
- Keep an eye on the mail for this package!
Step 3: Complete the Camp application and forms.
- Fill out the Camp application and forms.
- Return these forms via U.S. Postal Mail to your local American Diabetes Association with payment.
- Don’t delay in sending back forms — many Camps fill up quickly.
- Your application is processed in the order it was received.
Step 4: Check your mail for a confirmation notice and information packet from your local American Diabetes Association.
